To the patients who join the Chronic Kidney Disease Japan Cohort (CKD-JAC) Study:

Thank you for participating in the CKD-JAC Study.CKD-JAC study is planned to reveal the factors which lead patients with chronic kidney disease (CKD) to end stage kidney disease (ESKD) and the mechanisms that cause cardiovascular diseases (CVD) in CKD.

In Japan the number of ESKD patients on hemodialysis is more than 270,000 and is increasing by around 10,000 each year.

Several recent epidemiological studies have shown that CKD causes a decline of kidney function and that CKD patients have a high risk of CVD, such as stroke and myocardial infarction.

At present, though we are able to slow down the progression of CKD by controlling high blood pressure and by reducing urinary protein excretion, we still have a long way to stop the development and progression of CKD completely. In order to improve the treatment of CKD, we need further research on CKD and CVD.

Now we are planning to commence “the Chronic Kidney Disease Japan Cohort (CKD-JAC) Study”, and are asking for your kind cooperation with this project. The CKD-JAC study is being conducted on about 3,000 CKD patients nationwide for four years. CKD-JAC study is intended to investigate the actual medical and socio-economical situation of CKD study to find strategies to act against CKD. The aim of this study is not to find the effects of new treatments or new medicines. We hope that this study will be a great help for the prevention and better treatment of kidney disease and CVD in patients with CKD.

We appreciate your kind cooperation to provide us with information on your current condition so that this cohort study will provide important information on CKD. All the private information of the patients is strictly kept confidential and will not be used for any other purposes. However, the useful results we obtain from this study should be open to public and widely contribute to those who need them.
